Can drug used in leprosy (Clofazimine, Dapsone, Rifampicin and Prednisolone) cause hyponatraemia?

Leprosy Mailing List – August 29th, 2010

Ref.: Can drug used in leprosy (Clofazimine, Dapsone, Rifampicin and Prednisolone) cause hyponatraemia?

From: Ben Naafs, Munnekeburen, The Netherlands

Dear dr Idi,

It may be that I missed it. But I have never seen it. I also did a small internet search. I think that the internist has to look for another cause.

With kind regards.

From: Lysbeth Duncan, Peebles , Scotland , UK

Dear Dr. Noto,

I refer to Mr
Adamou Idi’s message dated LML Aug. 23rd, 2010. Having used the four drugs listed without seeing any evidence of hyponatraemia, personally I am not aware of this being a problem.

I do know that Rifampicin reduces the efficacy of prednisolone by 50%, as can oral contraceptives. I shall forward this letter to a colleague who is a leprologist and Microbiologist in case he can help further.

Best regards,

Lysbeth Duncan

Clofazimine in ENL reaction

Leprosy Mailing List – August 29th, 2010

Ref.: Clofazimine in ENL reaction

From: Ailton Santos Romero, Caruaru, Pernambuco , Brazil

Dear Dr. Noto

Drugs currently used in the treatment of reactions in leprosy are: non-steroidal anti-inflammatory drugs (NSAIDs), corticosteroids, and thalidomide. Thalidomide is the treatment of choice in erythema nodosum leprosum (ENL) or type 2 reaction, except for women of child bearing age.

In our centre, prednisolone and thalidomide, in men only, are the drugs of choice in ENL. In the past in patients with relapsing and chronic ENL reaction we have also used a regimen containing clofazimine. We found it useful in cases of steroid dependent ENL. The doses of clofazimine that we used were:

300mg, the first month
200mg the second month
100mg in the third month
50mg in treatment continued until remission of the clinical picture.

Unfortunately loose clofazimine is currently not available in our country.

I would like asking the experience of colleagues about the use of such a regimen, particularly about the time of regression of the reaction using clofazimine. We had excellent results with clofazimine, do others confirm this?

Thank you very much in advance for your collaboration.

My sincere compliments,

Dr. Ailton Santos Romero

Greetings from Infolep,

As you might or might not know Infolep is currently working on a new website. It should be ready later this year.

For the design of the website I would like to use photos of people from around the world using information resources (reading a book, sitting in a library, using a computer or using a mobile device or e-reader).

I would be very grateful if you could help me with this. You could send me existing photo's or make some new ones.

Many thanks in advance.

With kind regards,

Jiske Erlings

Can drug used in leprosy (Clofazimine, Dapsone, Rifampicin and Prednisolone) cause hyponatraemia?

Leprosy Mailing List – August 23rd, 2010

Ref.: Can drug used in leprosy (Clofazimine, Dapsone, Rifampicin and Prednisolone) cause hyponatraemia?

From: Adamou Idi , Niger

Dear Dr Salvatore

I greet you in the name of our Almighty God. Sir, let me to introduce myself to you, my name is Adamou Idi and I have been involved in the Leprosy Control Programme since 1993, I have been trained by many visitors like Dr Francoise Luthi.

We have a multibacillary leprosy patient on treatment, he is on at his fifth blister package, and is in severe leprosy reaction type 2 (ENL). He has been suffering with fever for more than 3 weeks, he has lost weight. The doctor that we are working with asked some questions about the sides effects of leprosy drugs like Clofazimine, Dapsone, Rifampicin and Prednisolone. Can they cause hyponotraemia? The patient has very low blood sodium.

We would greatly appreciate any advice.

With kinds regards,

Thank you

Adamou Idi
